What is a GAP Flap Procedure?

The GAP flap—short for Gluteal Artery Perforator flap—is a sophisticated breast reconstruction procedure that utilizes tissue taken from the buttock region. This procedure is ideal for women who do not have sufficient abdominal tissue for DIEP reconstruction and want to avoid implants.

The GAP flap surgery involves transplanting skin, fat, and blood vessels from the buttock area to form a new breast mound. This technique preserves the buttocks’ natural shape and appearance while sparing the gluteal muscles. By not utilizing these muscles, patients often recover more quickly and comfortably and maintain strength in the donor area.

Understanding the Two Main Types of GAP Flap Surgery

While the GAP flap procedure always involves transferring tissue from the buttock area, there are two distinct types of GAP flap surgeries available: the Superior Gluteal Artery Perforator (SGAP) flap and the Inferior Gluteal Artery Perforator (IGAP) flap. The primary difference between these two procedures lies in the location of the donor tissue.

SGAP Flap (Upper Buttock)

The SGAP flap procedure uses tissue from the upper portion of the buttock. This area generally provides sufficient tissue with a reliable blood supply and minimal disruption to daily activities, making it an excellent option for women seeking natural results.

Key points about the SGAP flap include:

  • Incisions are placed in areas typically concealed by underwear or swimwear.
  • Minimal impact on daily activities.

IGAP Flap (Lower Buttock)

The IGAP flap involves tissue from the lower buttock area, specifically within the natural crease below the buttock. Similar to the SGAP procedure, this method does not involve muscle removal, but it is performed less frequently today, having largely been replaced by alternative methods such as the PAP flap.

Important considerations for the IGAP flap include:

  • Incisions are placed within the lower buttock crease.
  • May cause discomfort while sitting due to the location of the incisions.

  • Surgery and reconstruction of the other breast to achieve symmetry.
  • Prostheses and treatment for physical complications associated with mastectomy, including lymphedema.
